Conflict of Interest Questionnaire

Johnson Controls Ethics Policy and the Conflict of Interest Policy requires that all employees disclose any of their existing or potential conflicts of interest. Employees should use this online questionnaire to report any potential or actual personal conflicts to the Company.





Gift Reporting Form

Employees should use this form to request approval for gifts given or received. Johnson Controls Ethics Policy and Third Party Travel, Gifts and Entertainment Policy require that Johnson Controls employees report any gifts over $200 USD received by the employee or given by the employee to a third party. In all cases, the granting or receipt of gifts, meals or entertainment must comply with applicable industry codes and local law. Gifts received from or given to Government entities should be reported using the AB Form below.





AB Form - Interaction with Government Officials

This form should be completed at least 7 days prior with any interaction with government officials that involves meals, entertainment, travel or lodging in excess of $20 USD per person or over $200USD per event or as required by local laws or regulations. Gifts, meals, entertainment, travel and lodging provided to government officials must comply with Johnson Controls Anti Corruption Policy and the Third Party Travel, Gifts and Entertainment Policy.

In accordance with the Third Party Travel, Gifts and Entertainment Policy, this form must be completed for each requested Company paid travel of a Commercial Customer if greater than USD 1,000 per person.
This form is also required if the Customer visit requires a Visa Invitation Letter issued by Johnson Controls.
For visit or visa requests involving a Government entity, the form must be completed regardless of travel value and submitted for approval at least two weeks prior to the Government entity’s proposed departure date.
Approval will not be granted nor will Visa Invitation Letters be provided unless questions are answered in full and supporting documentation is provided. All approvals must be obtained in accordance with the requirements described in the form. The completed form, all required approvals and any supporting documentation must be submitted as instructed in the “Return Completed Form” section below.

Charitable Contributions and Charitable Event Sponsorships Approval Request Form

In accordance with the Charitable Contribution and Charitable Event Sponsorships Policy, this form must be completed if:
a) The contribution / sponsorship amount is greater than $10000, or
b) The contribution / sponsorship (in any amount) has been requested by a Government Customer OR the Beneficiary is affiliated or associated with a Government Customer.

All approvals must be obtained in accordance with the requirements described in the form. The completed form, all required approvals and any supporting documentation must be submitted as instructed in the “Return Completed Form” section below.

Direct Corporate Political Contributions Form

In accordance with the Political Contributions Policy (140-100), this form must be completed for each requested Direct Corporate Political Contribution. The Requester may not make the Direct Corporate Political Contribution until he/she has received confirmation that the Direct Corporate Political Contribution has been approved by all Approvers as required by the relevant Approval Matrix and the Government Relations Approver. Retroactive requests for Direct Corporate Political Contributions will not be permitted or approved.

All approvals, as per the Delegation of Authority (DoA), must be obtained before returning this form for review. The employee should submit proof of all required approvals and any supporting documentation together with the completed form.
